Comparison of a Radical Prostatectomy in Men With and Without Prior Transurethral Resection of the Prostate

Introduction- To determine if there are any measurable differences when performing radical retropubic prostatectomy in men with and without prior transurethral resection of the prostate (TURP) Methods- A single surgeon performed 210 consecutive open radical retropubic prostatectomies between January 2000 and January 2005. Of these patients, 52/210 had undergone TURP at least six months prior to radical surgery and 158/210 patients did not. Both groups were compared in terms of estimated blood loss, margin positivity, continence at one year, operative time, and specimen weight. Results- In comparing patients that did not have a previous TURP versus those that did have a prior TURP, there was no significant difference in the estimated blood loss (780 cc versus 800 cc), margin positivity (18% versus 17.5%), or continence rate at one year (88% versus 89.5%). However, there were significant differences noted inoperative time (168 minutes versus 220 minutes) (p<0.05), and specimen weight (48 grams versus 29 grams) (p<0.05). Conclusion- When comparing radical retropubic prostatectomies performed in men with and without prior TURP, the only significant differences were the operative time and the specimen weight.
